HMO and Medicare
For patients with HMO insurance or Medicare, we need a referral from your primary care physician. Please ensure that you give them our fax number (432-224-1091) to ensure your referral gets to us properly and timely.
PPO Insurance and Self-Pay
For PPO insurance and Self-Pay patients, please book with us through Healow.
Our Self-Pay rates are as follows:
New Patient Self-Pay Rate: $300
Exisiting or Follow-Up Patient Self-Pay Rate: $150
Joint Injections: $40
